02No provider–patient relationship is created online

Reading this website, downloading a resource, or sending a message through the contact page does not create a provider–patient relationship. That relationship begins only after you have formally enrolled as a patient, completed the required intake and consent forms, and had a scheduled consultation. Until then, no clinical relationship exists and nothing here should be treated as personalized care.

03Services are available in Florida only

Irena Y. Ghen, MSN, APRN-BC is licensed to provide clinical services to patients physically located in the State of Florida. If you are located outside Florida, you may read this website's educational content, but you are not eligible to become a patient of this practice. You agree to be truthful about your location, and the practice reserves the right to decline or discontinue services if eligibility requirements are not met.

04Telemedicine & its limitations

This is a 100% virtual practice. All consultations are conducted by phone, video, or secure messaging; there are no in-person visits and no physical examinations. Telemedicine improves access and convenience, but it has limits you should understand:

• In rare cases, information transmitted may be insufficient (for example, poor image resolution), which can affect clinical judgment.
• Delays in evaluation or service can occur due to technology or connectivity failures.
• A virtual visit cannot replace hands-on examination where one is clinically necessary; in such cases you may be referred to an in-person provider.

By becoming a patient, you acknowledge these limitations and consent to the use of telemedicine for your care, as further detailed in the informed-consent documents you complete at enrollment.

05This practice does not handle emergencies

06Insurance & payment

This is a cash-based (out-of-network) practice. We do not bill insurance, we are not contracted with any insurance plan or Medicare, and membership fees are not reimbursable through us. When clinically appropriate, we can provide lab or imaging orders that you may run through your own insurance at outside facilities such as LabCorp, Quest, or BioReference; any charges from those facilities are billed by them, separately, and are your responsibility. Certain specialty functional tests are not covered by insurance and are billed separately.

07Fees, membership & refunds

• Discovery call: the discovery-call fee is due at the time of booking. Because time is reserved specifically for you, this fee is non-refundable, except that appointments cancelled or rescheduled with at least 24 hours' notice may be credited toward a rescheduled call at our discretion.
• Membership: concierge membership is an annual commitment billed as described at enrollment. Membership fees are non-refundable once the membership term has begun and services have been made available, except where required by law.
• Missed appointments: appointments missed without notice, or cancelled with less than 24 hours' notice, may be forfeited.

Specific current fees are provided to you in writing before you commit. Nothing on this website constitutes a bill or a request for payment; all payments are arranged directly and securely with the practice.

08Prescriptions & controlled substances

Where clinically appropriate, the practice may recommend or prescribe medications, including bioidentical hormone therapy, as well as nutritional supplements and other natural substances. The practice does not prescribe Schedule II, III, or IV controlled substances (such as opioids or benzodiazepines). Any prescription is issued solely at the provider's clinical discretion following appropriate evaluation, and no prescription is guaranteed.

09Labs & your medical records

Medical records, lab results, and imaging are exchanged only through secure, encrypted, HIPAA-compliant channels — never through standard email. When we need to send you an order, or you need to send us existing results, we use a secure provider platform and will guide you through it. Please never send protected health information by regular email. You are responsible for providing accurate, complete health information; clinical recommendations depend on it.

10Privacy & HIPAA

We are required by law to protect the privacy of your health information and to notify you of our legal duties and privacy practices. A copy of our Notice of Privacy Practices is available to you upon request and is provided during enrollment. By enrolling, you consent to our use and disclosure of protected health information as described in that Notice. You may revoke that consent in writing at any time, though revocation does not affect disclosures already made in reliance on your prior consent. This website itself does not knowingly collect protected health information through public forms.

11Supplements & FDA notice

Any nutritional supplements referenced or recommended are intended to support general wellness. These statements have not been evaluated by the Food and Drug Administration. Products mentioned are not intended to diagnose, treat, cure, or prevent any disease. Individual needs and responses vary. Consult your provider before beginning any new supplement, particularly if you are pregnant, nursing, taking medications, or managing a health condition.

12No guarantee of results

Functional and integrative medicine works with your body's own systems, and outcomes vary from person to person. We do not and cannot guarantee any specific health result, improvement, or outcome. Any examples, testimonials, or general information reflect individual experiences and are not a promise that you will experience the same.

13Intellectual property

All content on this website — text, graphics, tools, downloadable guides, and design — is the property of Irena Y. Ghen / Integrative Medical Consulting, Inc., unless otherwise noted, and is protected by copyright and other laws. You may view and use it for your own personal, non-commercial reference.
